Request Medical Records

To request medical records, please call the Health Information Management Department at 641-842-1478. To protect our patient's privacy, medical records cannot be faxed directly to the patients. Patients will be required to pick up their records in person from the Health Information Management Department and sign a release form. This is to protect our patient's privacy and security.

Five Wishes is an easy to use legal document that helps families plan for serious illness. "It is unique among all other living will and health agent forms because it looks to all of the person’s needs; medical, personal, emotional and spiritual.
Five Wishes lets your family and doctors know:
1) Which person you want to make healthcare decisions for you when you can't make them
2) The kind of medical treatment you want or don't want
3) How comfortable you want to be
4) How you want people to treat you
5) What you want your loved ones to know
